Introduction

To complete the annual club affiliation process, you will need to submit the following information, documents and fees. Make sure to have these available to you before continuing the process.

1. Updated club contact information of current officers and administrators.
2. Proof of any of the following:
   1. Current registration of your corporation with the appropriate state agency.
      
   2. Current registration of your parent organization with the appropriate agency and an updated certificate of compliance.
      
   3. Proof of continued government affiliation with updated certificate of compliance.
3. Current copies of Articles of Incorporation and Bylaws.
4. Current club development plan.
5. Annual Club Affiliation fee of $350.
